Before You Begin
In preparing your memorial, you will find it helpful to gather the following information
beforehand:
- Birth Date and Birthplace

Date of Passing and Cause of Death
- Day and
Time of Funeral and/or Memorial Services
- Reference information:
- Names of parents,
siblings, spouse, children, and other relatives
- Schools attended
- Special achievements

Important dates (wedding anniversaries, retirement dates, etc.)
- Armed forces service information
- Church affiliations or other important affiliations
You may also want to include:
- Special writing or poem
- Up to two digital photos (72 dots per inch and
